Superior Vision - General Information

The initial enrollment period ends April 25, 2008. All benefits are effective April 1, 2008, even if you turn in your paperwork after that date. If your enrollment is not received prior to payroll processing in mid April, you will be billed two months premiums in May.

Your next opportunity to enroll in the vision plan will be during the Open Enrollment Period in the Fall of 2008.
